What and How to Upload:

Which page? Within the physical, there is a page titled, "Medical Eligibility Form", and it's the only page of the physical required for registration, (do not submit the other pages of the physical).
The medical eligibility form page must clearly display the participant's name, date, box checked indicating participation approval/disapproval or with details of restrictions, and the medical practitioner's signature. Example below.
Expiration: You can resubmit a previous physical if it's valid through the current season (physicals are valid for 12 months from issue date). A valid physical must be uploaded each season.
Upload Instructions: Take a clear picture of the completed page within the physical titled "Medical Eligibility Form" and upload it from this screen using the detailed instructions and images below. Complete "checkout" ($0). Do not upload multiple pages. Instead, upload ONLY the medical eligibility form page shown and detailed below.
